Don't quite feel up to a full annotation at the moment but I felt obliged to point out move 42 was a bit of a shocker and the reply even worse. Unless I'm missing something, the white queen seems undefended and the white bishop is pinned. Hence, the errant white rook on e6 is not defended and the black queen could have snaffled it. Or not?

It's a bit too tactical for me at first glance. I'm wondering, though, if black had better thematic KID options earlier in the game. In particular, at move 9. ...a5 seems odd and it's not clear what this achieves. Now it looks like you were following the only database game in this position, here, when black played a5 and went on to win - but this is due in part to white's 14.f4? which allows black to go on a strong attack with his open lines and good Indian bishop (the one on g7).

Instead of 9. ...a5, you could look at playing 9. ...c6 or trying to prepare ...f5 with, say, 9. ...Nfd7. The game is obviously completely different in this line, which means that I can't say what would happen from then.
